The mission of District 104 is to promote and instill the love of learning. Our goal as a health team is to create a healthy and supportive environment that facilitates optimal learning and development for all students. Each district 104 school is staffed by a full-time registered nurse.

Accommodations for Medical Conditions & Injuries:

Any students requiring specific accommodation in the school setting for medical conditions and/or injuries require a recent doctor’s note on file in the Health Office indicating the provider’s recommendations. PE excusals must be written by a physician and submitted to the Health Office and PE teacher.

Medication Reminder:

Students are NOT allowed to carry ANY medication (this includes any over-the-counter medication such cough drops, Tylenol, and Motrin). If your student needs medication administered during the school day (including cough drops, Tylenol, or Motrin), medication must be kept in the Health Office and medication authorization forms must be signed by the parent/guardian AND A MEDICAL PROVIDER (Doctor, PA, APRN). Students may have the option to self- carry emergency medications (Glucagon, EpiPen, Rescue inhalers) if noted on the medication authorization form.
